Our office is committed to protecting the privacy of the personal health information collected from our clients and patients. We recognize that to maintain your trust, we must be open and accountable in how we safeguard the information you share with us.
This policy ensures that your Personal Information (PI) and Personal Health Information (PHI) are kept confidential. We operate in strict accordance with Ontario’s Personal Health Information Protection Act (PHIPA).

Personal Health Information (PHI): Any information that connects an individual to the receipt of care at our office. This includes physical or mental health status, diagnoses, testing, treatment history, family health history, and details regarding your Substitute Decision Makers or care providers

We collect PHI directly from you or your Substitute Decision Maker. This may include:

Information from other sources, provided we have obtained your consent or the law permits.

We use your personal health information to:

Determine eligibility for services and identify appropriate programs.

Provide, coordinate, and consult with other healthcare providers regarding your care.

Plan, evaluate, and manage our internal services and programs.

Conduct risk management, quality improvement, and system planning.

Comply with regulatory requirements, such as reporting to the Ministry of Health and Long-Term Care.
4. Disclosure of Information (Sharing)
We obtain express consent (verbal or written) before sharing your information with external parties, including referrals. However, PHIPA permits or requires us to disclose information without consent in specific circumstances:

Safety: In life-threatening situations or emergencies.

Legal/Regulatory: To the Ministry of Health, the Medical Officer of Health (communicable diseases), or the Workplace Safety and Insurance Board (WSIB).

Protection: To the Children’s Aid Society if child abuse is suspected.

Legal Proceedings: To law enforcement with a warrant/subpoena, the Coroner, or the Public Guardian and Trustee.
5. Your Rights
Under PHIPA, you have the following rights regarding your information:

Withdraw Consent: You may withhold or withdraw consent for the use or disclosure of your PHI at any time (subject to legal notice).

Access & Copy: You have the right to view and receive a copy of your health records.

Request Correction: You may request that we correct inaccurate or incomplete information in your file.

File a Complaint: You may raise concerns with our Privacy Officer or contact the Information and Privacy Commissioner of Ontario (IPC).
6. Website & Digital Privacy
Cookies and Tracking
Our website uses “cookies” to remember your preferences and improve your user experience. These cookies contain anonymous, non-personal information. We also maintain log files to track website traffic (IP addresses and browser types) to help us improve our site content. Individual identities are not determined from these reports.
